RTOG HQ CCOP/Symptom Management Concept Review Form (all phase) – this template is used for all protocol concepts submitted under the NCI DCP CCOP program. This version of the concept form is used only after the Preliminary Concept (see above concept form) for the study has been approved by the RTOG CCOP/Symptom Management Steering Committee.

Treatment Concept Form for Phase I, I/II, or II < 100 Patients – this template is used for treatment protocols for Phase I, I/II or II trials with < 100 Patients.

Treatment Concept Form for Phase II >/= 100 Patients, II/III or III – this template is used for treatment protocols for Phase II trials of 100 patients or more, II/III, or III studies.
