Certification Worksheet for Neurocognitive Test Administrator (Appendix V in the protocol) MUST be completed and signed by the test administrator (ie, nurse, research associate, physician) requesting certification and MUST then be submitted to Dr. Caine prior to registering any patients on RTOG 0925.

Step 1: Obtaining Certification
Become certified for administering CogState assessments.
If you have already been certified, contact RTOG Headquarters at 0925cogstate@acr.org
 
Step 2: Obtaining CogState Software
Contact RTOG Headquarters at 0925cogstate@acr.org to obtain a USB Drive.
 
Step 3: Administering CogState Assessments
Refer to the RTOG 0925 Neurocognitive Tests Manual also available on the USB Drive.
