The following changes are in effect:

Eligibility Checklist - Added

Will any component of the patient’s care be at a VA or military facility?

Section 10.0 Updated the shipping address; additional case credit for pathology submission no longer applies (also affects Section 12.1).

Section 12.1 The M2 Form is no longer required; the P2 was added.

The following changes are in effect:

Section 6.2.4.2 Change to

" . . . may not exceed 5 mm."

Section 6.2.6 The first sentence in both Sections 6.2.6.3 and 6.2.6.4 was extended.

" . . . receiving dose in 1 or 2 Gy dose intervals . . . should additionally be reported".

Appendix I, (p. 18, Risks) The risks of radiosurgery are more clearly defined.
