The following changes (SWOG Revision #4) are in effect:
1. References to "CGOP" have been replaced by "Affiliate(s)" in the title page and in Section 14.3a.
2. The title "High Priority Medical Oncologist" has been removed from the participant list on the title page since this program is no longer active.
3. Dr. John F. Ensley's contact information has been updated on the title page and in Section 16.23.
4. James Moon. M.S. has replaced Danika Lew, M.A. as the primary statistician on this study. His contact information has been added to the title page.
5. The word "Member" was added to the title of Section 14.3a. Additionally, the CCOP institution's instructions and mailing address have been added to this section.
6. ECOG Institutions participating on Southwest Oncology Group studies will now submit data directly to the Southwest Oncology Group Statistical Center rather than routing it through the ECOG Coordinating Center. Data submission instructions for ECOG institutions have been revised as specified below.
14.3b has been revised to read "ECOG Institutions: A copy of the eligibility checklist must be submitted to the Southwest Oncology Group Statistical Center at the address below. Include both the ECOG and Southwest Oncology Group study number and patient number.
