The following changes are in effect:

Cover Page Dr. Fu's phone and fax number were updated.

The Eligibility treatment period has been extended to include patients treated through 1996; chemotherapy is also permitted. This affects Sections 1.0 (last paragraph), 2.0, 3.2, 3.3-5 (new), 5.2, 6.2, 6.3 (new), 7.0 (new), 10.1.3, 10.2.8, 12.1, 13.1, 13.2 (last paragraph), 13.3, 13.4, REFERENCES 47-49 and Appendix IV (Q5).

All materials will be sent to LDS Hospital. This affects Section 10.1.2, 12.2, and the Pathology Submission Form.
