The following changes are in effect:

Section 3.2.5 Changed to

Section 7.1 Bristol-Myers-Squibb (BMS) will notroutinely provide taxol. In instances of extreme hardship, apply to BMS for patient-specific consideration. This change affects Secions 7.1.1-2.

Section 7.1.6.2 Added

Individual treating physician may substitute Ranitidine, Famotidine, or oral dexamethasone at their discretion.

Section 7.2.7 (new) Modifications for neuropathy were added.
