  • Data to be submitted to QA office within one week of start of radiation treatment:


  • T2 - Protocol treatment form
  • T3 - Initial simulation / port films
  • T4 - Radiation dose calculation data form

  • Retrospective evaluation of the entire course of radiotherapy
  • Assess protocol compliance and quality of delivered treatment
  • Emphasis on total dose delivered, field border placements, applied fractionation and total elapsed days of treatment

  • Data to be submitted to QA office within one week of completion of treatment:


  • T5 - radiation therapy treatment record
  • T6 - summated isodose distributions
  • T8 - simulation/port films for all cone down fields
  • T1 – radiotherapy form
